Patient Safety

As many as 98,000 Americans die each year because of medical errors. Others suffer disability or permanent functional impairment. And hundreds of thousands more experience the risks associated with tests, procedures and hospitalizations that otherwise would not have been necessary.

VHQC consults with hospitals and nursing homes to eliminate opportunities for patient harm. Through the Patient Safety Learning Network, we are focusing on five topics for which changes to systems of care can yield big improvements in quality and reliability.

Our approach combines on-site consultation by expert staff, ongoing email and phone support, convenient webinars and teleconferences, and group events where participants learn best practices and share successes. All facilities also receive a comprehensive change package that includes valuable improvement tools, references and resources, plus periodic data reports measuring their progress.
