Prevention

Medicare now pays for more preventive benefits than ever; however, few enrollees are taking full advantage of them. Statistics show that while Medicare consumers visit the doctor six or more times a year, many of them are not aware of their risk for disease—or that they may already have a serious condition a simple test could detect.

VHQC is consulting with selected physician practices to leverage the power of electronic health records to increase rates of mammography, colorectal screening and immunizations. These practices are learning how they can modify clinical workflows to achieve greater efficiency, fully utilize EHRs to generate the highest return on investment, and extract and report data from their systems for success in future pay-for-performance programs. In addition, they are taking advantage of EHR functionality to:

  1. Track preventive care, tests and immunizations
  2. Generate clinical reminders at the point of care
  3. Identify patients who have not received preventive services
  4. Trigger alerts about potential drug interactions
  5. Measure preventive service rates at the practice level
  6. Report performance on preventive care and quality of care indicators
