Outpatient/Physician Office Setting
VHQC consults with practitioners in the outpatient setting to decrease the incidence of drug-on-drug interactions and the prescription of potentially inappropriate medications. This is of particular challenge in this setting as patients may be seeing multiple prescribers and using multiple pharmacies. This often results in incomplete information at the time of prescribing and dispensing outpatient prescriptions. In addition, the more practitioners learn about potentially inappropriate medications, the better they can apply this knowledge to their patients’ medication regimens.
